Mike Maples, MD - Chief Executive Officer
Dr. Maples enjoys the full scope of family medicine, caring for
families throughout the life cycle. He is committed to patient education,
to help patients understand their wellness needs, and to make informed
decisions regarding their illness care. He is a graduate of the
University of Washington School of Medicine. He did his family medicine
residency at University Hospital in Seattle, completing that training
in 1983. After graduation he moved to Yakima, where he practiced
for six years. In 1989, Dr. Maples relocated to Seattle to assume
a faculty position at the Group Health Residency Program and completed
the UW Family Medicine Faculty Development Fellowship, learning
the skills needed to teach physicians in the specialty of family
medicine. In 1992, he returned to Yakima to become the founding
Director of the CWFM residency program. Dr. Maples was named CEO
of CWFM in 2003.

Vicki Black, MD - Medical Director
A board certified graduate of Loyola School of Medicine and the
Family Medicine Residency in Oak Park, Illinois, Dr. Black practiced
for fourteen years in rural Washington with medically underserved
populations. She is skilled in the practice of obstetrics and special
procedures, and is a national advisory faculty for Advanced Life
Support in Obstetrics (ALSO). Dr. Black completed the Family Medicine
Faculty Development Fellowship at the University of Washington with
research on Women in Rural Medicine. She is a past president of
the Washington Rural Health Association and the Washington Academy
of Family Physicians. Dr. Black served as Associate Director of
CWFM's residency program from 1992-2003 and Program Director from
2003-2005.

Swapna
Bobba, MD
Dr.
Bobba was interested in medicine, even as a child. While at Kakatiya
Medical College, she learned first hand how experience can augment
academic learning. After graduating from medical school, Dr. Bobba
moved to the United States and completed two years of an anesthesiology
residency program. She transferred to family medicine because she
missed the breadth and depth of patient care, particularly care
of the family unit.

Roger
Bracchi, MD
Originally
from Portland, Oregon, Dr. Bracchi is a board certified graduate
of Oregon Health Sciences University and the prior family medicine
residency program in Yakima. He was in private practice in Yakima
for 13 years before joining CWFM. Dr. Bracchi was formerly chair
of the Department of Family Medicine for Yakima's conjoint medical
staff. He staffs the travel medicine clinic at CWFM in addition
to practicing full spectrum family medicine including obstetrics.

Yasmin
Aslam Chaudhry, MD
Born
in Sialkot, Pakistan, Dr. Chaudhry is the third member of her immediate
family to become a physician. After graduating from King Edward
Medical School, she pursued her love of family medicine, enjoying
the diversity of patient care. She especially enjoys pediatrics
and obstetrics.

Tanny
Davenport, MD
A University of Washington School of Medicine graduate, Dr. Davenport
was born in Seattle and raised in Wapato. He always knew he wanted
to be a doctor; as a high school freshman, Dr. Davenport had his
first opportunity to observe a cesarean section and knew he belonged
in medicine. In college, he was intrigued by the subjects within
his major of chemistry-biology, but found fulfillment in volunteer
activities. Medicine is the perfect combination of his love of science
and his desire to serve others.

Robyn
Davies, MD
Originally
from
Tacoma, Washington, Dr. Davies received her Bachelor of Science
in Biology at Pacific Lutheran University, a Masters in Biomedical
Sciences at Barry University in Miami Shores, Florida, and her medical
degree from St. George's University in Grenada. She believes in
building a foundation of trust, respect, and compassion with her
patients and is committed to being a doctor who treats the person
and not just the symptom or disease.

Mark
Deramo, MD
Originally
from Youngstown, Ohio, Dr. Deramo received his medical degree from
Georgetown University in Washington, D.C. He graduated from the
University of Pittsburgh Medical Center (UPMC) Shadyside Family
Practice Residency Program, and in June 2004 completed a fellowship
in Faculty Development at UPMC McKeesport. Dr. Deramo emphasizes
holistic medicine in his medical practice.

Payal
Gaba, MD
Ever
since Dr. Gaba decided on a career in medicine, she always wanted
to be in family medicine. During her medical school and subsequent
internship experiences at Lady Hardinge Medical College, her rotations
in medicine, pediatrics, gynecology, OB and surgery solidified her
resolve. While completing her internship, Dr. Gaba volunteered for
the National AIDS Control Organization and was the medical officer
in charge of the subcenter for the National Pulse Polio Immunization
Program. She desires to provide compassionate and technically excellent
medical care to patients from all walks of life.

Jose
Garcia, MD
As
a little boy growing up in Cuba, Dr. Garcia knew he was going to
become a physician someday. His father, sister and several other
members of his family are physicians in Cuba. He received his medical
degree from Instituto Superior de Ciencias Medicas. The difficulties
he faced when trying to flee Cuba and his early experiences in the
U.S.A. didn't douse his deep passion for medicine. While applying
for residency, Dr. Garcia was employed as a medical assistant in
a family medicine clinic. He is a strong advocate of preventive
medicine and appreciates the close relationship between patient
and physician.

Debra
Gould, MD, MPH
Originally
from rural Canaseraga, New York, Dr. Gould received her medical
degree from the State University of New York at Buffalo. She completed
her residency in family medicine at Highland Hospital, University
of Rochester, New York. She went on to receive a Masters degree
in public health and complete a 2-year fellowship in health services
research at the University of Washington, Seattle. Dr. Gould chose
family medicine because she enjoys the process and complexity of
"working with the whole patient in the context of his or her
family and community."

Ralph
Havens, MD
Originally
from San Diego, California, Dr. Havens has ancestral ties to the
Cherokee Nation. He learned to appreciate the richness of cultural
diversity as a child through international travel and experiences
during medical school in Mexico. Dr. Havens completed a Fifth Pathway
at New York Medical College, a Masters in Public Health at Loma
Linda University, and an internship in general surgery at New York
Hospital, Queens. He is fluent in Spanish.

Daniel
Jereza, MD
Dr.
Jereza dreamed of being a doctor since his boyhood days in the Philippines.
After receiving a Bachelor of Science degree in Biology, he enrolled
in West Visayas State University Medical School. He served as Medical
Director of a medical and diagnostic center in Makati City and a
company physician in Micronesia. In 1999, he immigrated to the United
States, graduated from the United States Naval Hospital Corps School
and served in a family medicine clinic at the Naval Air Station
in Lemoore, California. In May 2004, Dr. Jereza was commissioned
by the U.S. Army.

Portia
Jones, MPH
Born
and raised in Seattle, Washington, Dr. Jones received a Bachelor
of Arts in Spanish from George Fox College in Newberg, Oregon. She
attended Hahnamann University School of Medicine for two years before
transferring to the University of Minnesota School of Medicine,
where she received her medical degree. She simultaneously completed
her residency in family medicine and a fellowship in obstetrics
at Franklin Square Hospital in Baltimore, Maryland. Dr. Jones previously
held academic medicine appointments in Maryland, Minnesota and New
York. She completed a Masters of Public Health at Johns Hopkins
School of Hygiene and Public Health and is a certified instructor
for Advanced Life Support in Obstetrics and the Neonatal Resuscitation
Program.

Kavitha
Krishnamani, MD
As a child, Dr. Krishnamani cared for her sick cousin and remembers
the joy she felt when her cousin’s condition improved. From
that moment, Dr. Kirshnamani knew she always wanted to be a physician.
Early on her resolve to become a physician led her to compete in
a nationwide entrance exam to secure a scholarship to India’s
prestigious Madras Medical College in Chennai. During her second
clinical rotation, Dr. Krishnamani discovered she did not want to
be restricted to just treating the heart or lungs, but rather to
care for the patient as a whole person. This experience, along with
many others, led her to choose a career in family medicine.

Mohit
Mahajan, MD
Born
and raised in northern India, Dr. Mahajan learned good quality work
ethics and self-discipline when he joined the Sainik (military)
School in the sixth grade. He came to respect the campus doctors,
whose integration of compassion and understanding, along with their
skill of healing, inspired him to pursue a career in medicine. When
the opportunity to attend medical school appeared, Dr. Mahajan readily
accepted it and enrolled at the Krishna Institute of Medical Sciences.
After completing his medical education, Dr. Mahajan followed up
with an internship at the Indira Gandhi Medical College and Hospital.
He later immigrated to the United States and worked as a medical
assistant to become familiar with the U.S. healthcare system.

Russell
G. Maier, MD
Dr. Maier is a graduate of the University of Washington School of
Medicine and Family Practice Residency where he served as Chief
Resident. He chose family medicine because it offers the opportunity
of a close relationship with patients. He completed the University
of Washington Faculty Development Fellowship in Family Medicine
and is board certified. Dr. Maier is the Director of CWFM's residency
program. He actively promotes community health initiatives and research,
particularly oral health in primary care.

Mike
Mandzuik, MD
The path leading Dr. Mandzuik to family medicine spans
ten years, five countries, four languages and countless unforgettable
people. While teaching English in the Ukraine, friends planted the
idea of a career in healthcare. He graduated from the International
Health and Medicine program at Ben Gurion University of The Negev
in Israel, where he completed core clinical rotations in Israel
and electives in Argentina, the Peruvian Amazon and the United States.
Dr. Mandzuik has a deep and abiding commitment to family medicine.

Meera
Mohanakumar, MD
Born in Colombo, Sri Lanka, Dr. Mohanakumar received her medical
degree from the University of Jaffna, Sri Lanka and continued her
medical experience as a licensed medical officer in an outpatient
unit where she treated and counseled a variety of patients. When
the Central Bank of Sri Lanka was bombed, she and her team treated
hundreds of critically injured patients. Dr. Mohanakumar moved to
the United States to further her education in family medicine, with
the goal of becoming a counselor, leader and friend to her patients.

Camille
Pajarillo, MD
Dr. Pajarillo’s Dad inspired her to become a physician. As
a child, she tagged along when he visited the homes of his patients
and observed his nurturing medical care. She loved what she saw:
the interaction between doctor and patient and treating the whole
person, not just the disease. It was with great excitement that
she attended Far Eastern University medical school in the Philippines.
After her internship in 2000, she took the medical board exam, became
a licensed physician, and joined her family in the United States.
Dr. Pajarillo enjoys being able to serve her patients.

Neema
Reddy, MD
After graduating from JJM Medical College in Devangere, India, Dr.
Reddy's training in a hospital in India with limited resources and
a large patient volume instilled within her a commitment to family
medicine. She learned to manage both in- and out-patient services,
performed routine medical procedures, and practiced OB. After moving
to the United States in 1999, Dr. Reddy continued her family medicine
training in Oregon before relocating to Yakima.

Jacobo
Rivero, MD
Originally
from Mexico, Dr. Rivero received his medical degree from the Universidad
Autonoma Metropolitana in Mexico City and completed his residency
at Family Medicine of Spokane. Following residency, Dr. Rivero moved
to Carlsbad, New Mexico, where he worked as a family medicine physician,
an emergency room physician, and the Chief of Medical Staff. He
is especially interested in geriatric medicine, hospice/palliative
care, genetics, and medical ethics.

Ricardo
Rois Romero, MD
Originally
from Colombia, Dr. Rois became interested in pursuing a career in
medicine after working alongside the Red Cross during his obligatory
year in the Colombian army. In 1993 he enrolled at the University
of El Bosque. After graduation, he worked for the Columbian government
as a general practitioner. During this time he came to feel that
the health care system in his country was not sufficiently prepared
to address the diversity of patient needs. Therefore, he came to
the United States not only to gain medical knowledge, but also to
seek guidance as a healthcare professional.

Laila
Sajwani, MD
During her various specialty rotations at Liaquat Medical College
in Pakistan, Dr. Sajwani discovered that family medicine best complemented
her career goals. She believes that family medicine plays a unique
and vital role in educating families and providing preventive care
by attending to the diagnoses and treatment of a broad range of
health issues. After graduating from medical school, she completed
an internship at Aga Khan University Hospital and then practiced
as a family physician in Pakistan. In 2000, Dr. Sajwani moved to
the United States to pursue postgraduate training in family medicine.

Teerink, DO
Originally from Redmond, Washington, Dr. Teerink received a Bachelor
of Science in Biology from Gonzaga University. She graduated from
Kirksville College of Osteopathic Medicine and completed her internship
year at the Genesys Family Medicine Residency Program in Michigan.
In addition to full-spectrum family medicine, Dr. Teerink's interests
are women's and children's health care.

Teerink, DO
Dr.
Teerink received a Bachelor of Arts in Biology-Health Professions
from Northwestern College, Orange City, Iowa. He graduated from
Kirksville College of Osteopathic Medicine and completed his internship
year at the Genesys Family Medicine Residency Program in Michigan.
Dr. Teerink's area of interest, in addition to family medicine,
is Sports Medicine.

Selemani
Wambuzi, MD
Dr.
Wambuzi was introduced to family medicine when he journeyed to Stockholm,
Sweden, for an international health conference. He was immediately
intrigued by the thought of following a patient from birth to death
while caring for an entire family. Following medical school at the
University of Zambia, Dr. Wambuzi moved to the United States to
pursue residency training in family medicine. He is proud to be
the first Zambian family medicine physician.

Originally
from Patagonia, Arizona, Debbi Spitler moved to Washington to complete
the Physician Assistant program at the University of Washington.
What she likes most about being a physician’s assistant is
the personal interaction she has with patients of all ages and backgrounds.
She’s fascinated by the unique characteristics of each presentation
and the way individuals respond to prevention and treatment plans.

Born in Lake Lillian, MN, Michelle Svihel completed the Physician
Assistant program at Augsburg College, Minneapolis, MN. She enjoys

Lemp, DO
Dr.
Lemp
came to CWFM with five years experience as an attending physician
in inner city St. Louis, fulfilling her obligation with the National
Health Service Corps. Working with a poor and medically underserved
population has always been a focus in her pursuit to practice pediatric
and adolescent medicine. That commitment continues as Dr. Lemp dedicates
herself to the current medical needs of the many cultures in the
Yakima Valley. She is a volunteer medical advisor for Children’s
Village and Carroll Children's Center.

Stephen
Pearson, MD
Originally
from the east coast, Dr. Pearson has lived in the Yakima Valley
since 1989. He has provided pediatric care to low-income and minority
populations in community health centers and medically underserved
communities and enjoys teaching and research applicable to ambulatory
practice settings. Dr. Pearson feels that family medicine physicians
and pediatricians can be involved in research to further evidence
based knowledge and improve the overall health of adults and children.
He is chair of the national steering committee for Pediatric Research
in Office Settings.

Marvin
Scotvold, MD
Dr. Scotvold is a Clinical Professor of Medicine (dermatology),
and pediatrics at the University of Washington School of Medicine,
and the Executive Director of the North Pacific Pediatric Society.
He completed residencies in dermatology and pediatrics and did a
fellowship in pediatric endocrinology prior to a long tenure in
academic medicine. Dr. Scotvold staffs the monthly dermatology clinic
at CWFM.

Louise
Achey, PharmD
Dr.
Achey loves what she does. She received her Doctor of Pharmacy degree
from Idaho State University and has been a pharmacist for 26 years,
12 of which were spent in a hospital pharmacy and the past eleven
at CWFM. Her interests lie in pharmacokinetics and herbal products.
She staffs the anti-coagulation clinic at CWFM and co-coordinates
the Diabetic Patient Education program. Dr. Achey is a Clinical
Assistant Professor of Pharmacology with the Washington State University
College of Pharmacy and teaches in the MEDEX program at University
of Washington.
